SANCTIONED TOURNAMENTS FOR 2008 (see below for division links) The PWSA will again post all Sanctioned Tournaments on the PWSA online newsletter as well as on the PWSA web site and in the PWSA Bulletin. When you send in your tournament information, please use the Sanctioned Tournament Request Form as this makes it easier for us to post the following information. The cost will remain at $35.00 for as many tournaments as your association or team sanctions. Please make your cheques payable to “PWSA” and mail form and cheque toTodd Bannister Old Highway 24, #2174 R. This e-mail address is being protected from spam bots, you need JavaScript enabled to view it NOTE: It is of utmost importance that you must complete a Tournament Report Form after each and every tournament. This form must be filled out and returned to me at the above address. NOTE - VERY IMPORTANT INFORMATION: The insurance company who holds the policy for sports activities is getting very strict on their policies. It is of utmost importance that you have a supply of Accident Report Forms at each and every tournament in the event of an accident. When you sanction your tournament with the PWSA, an Accident Report Form and Sanctioned Tournament Report Form will be attached to the email. Please keep those attachments for your records. Tournament Report Forms shall be returned to the Sanctioning Chairperson within the specified period of time. Failure to submit a written report within four (4) weeks or failure to follow tournament guidelines will result in a fine of $100.00. Future sanctioning of tournaments by the offending team/league/organization will be withheld until such time as the fine is paid. NOTE: If you have a team who has borrowed a player from another team, the team must obtain a Letter of Permission from the team that the player is registered with. If they do not have this Letter of Permission, they will not be covered by insurance. A copy of this Letter of Permission must also be sent to the Sanctioned Tournament Chairman. It cannot be stressed how important this is and we suggest that this point be added to your Tournament Rules.
